Job summary

SalusCare, Inc. is seeking a compassionate Discharge Planner with clinical experience in multi-campus care for patients with mental health and addiction diagnoses. You will coordinate and facilitate the development of care plans and ensure accurate discharge information across Acute Care settings.

This role requires crisis intervention skills, meticulous documentation, and collaboration in adult and adolescent crisis and detox units. Must complete pre-employment screens and orientation.

Qualifications

• Requires a Bachelor’s degree in a human services field and 1 year of direct care experience in behavioral health.
• Or a Master’s degree in a Human Services field; experience providing direct care services in a behavioral health setting preferred.
• Must be able to attain competency with the electronic health record within required timeframes.

Committed to providing a therapeutic environment, meticulous documentation, and collaborative care in adult.children's crisis and detoxification units.

What you will do.

Job Responsibilities:

• Provide clinical and psychosocial services to Acute Care patients by coordinating and facilitating the development of care plans.
• Provides through and accurate discharge planning information.
• Maintains high standards of accuracy and completeness across all required clinician and administrative records.

Job Qualifications:
