Job summary

SalusCare, Inc is seeking a compassionate Discharge Planner with clinical experience in multi-campus care for patients with mental health and addiction diagnoses. The role focuses on crisis intervention, documentation, and collaborative care in adult and children’s crisis and detox units.

The candidate will coordinate discharge plans, ensure accurate records, and work within a mission-driven nonprofit serving Southwest Florida. Generous PTO and stable, community-based environment offered.

Qualifications

• Bachelor’s degree in a human services field plus 1 year of direct care experience.
• Or a Master’s degree in a Human Services field; direct care experience preferred.
• Must be able to attain competency with the electronic health record (EHR) system.

Responsibilities

• Provide clinical and psychosocial services to Acute Care patients by coordinating and facilitating the development of care plans.
• Provide thorough and accurate discharge planning information.
• Maintain high standards of accuracy and completeness across all clinician and administrative records.

Skilled in crisis intervention, mental health assessments and documentation. Committed to providing a therapeutic environment, meticulous documentation, and collaborative care in adult.children's crisis and detoxification units.

What you will do.

Job Responsibilities:

• Provide clinical and psychosocial services to Acute Care patients by coordinating and facilitating the development of care plans.
• Provides through and accurate discharge planning information.
• Maintains high standards of accuracy and completeness across all required clinician and administrative records.

Job Qualifications:

• Requires a Bachelor’s degree in a human services field from an accredited college or university in social work, psychology, counseling or related field and 1 year of experience providing direct care services in a behavioral health setting.
• OR a Master’s degree in a Human Services field; experience providing direct care services in a behavioral health setting preferred. Co-occurring experience preferred.
• Must be able to attain competency with the electronic health record and use it effectively within required timeframes.
