The role emphasizes initiative, documentation, and communication, with responsibility for liaison during hospitalization and supporting the care team to meet schedules and goals.

Qualifications

• High School Education/GED or equivalent: Required.
• Associate’s/Technical Degree or equivalent: Preferred.
• Experience in a healthcare setting: Preferred.
• Demonstrates initiative in managing the discharge/transition process.
• Composes confidential correspondence, reports and related material.
• Supports physicians by providing information to referral agencies and discharge readiness.
• Acts as discharge planning contact for patient and family during hospitalization.
