Job summary

PIH Health Downey Hospital is seeking an RN Case Manager to guide patients from pre-admission through post-discharge, ensuring smooth transitions and appropriate utilization of hospital services.

You will focus on maximizing reimbursement from third-party payors and educating staff about healthcare options, while maintaining high standards of patient and family engagement throughout the continuum of care.

Qualifications

• Maintain an active California RN license.
• Computer proficiency.
• Broad knowledge of disease processes and treatment plans.
• Strong assessment skills with advanced critical thinking and analysis.
• Familiarity with discharge planning and care options.
• One year of acute hospital nursing experience (preferred).

Responsibilities

• Manage a continuum of care from pre-admission through post-discharge for assigned patients.
• Ensure patients and families progress efficiently throughout hospitalization.
• Focus on appropriate utilization of hospital services and maximizing reimbursement from third party payors.
• Educate medical/hospital staff about healthcare options.
