Job summary

PIH Health is seeking a discharge coordination specialist to support the Care Management team by engaging patients and families and handling regulatory discharge documentation. You will assist with post-acute coordination and real-time electronic tracking to ensure timely, compliant discharge planning.

Under delegated authority from the RN Case Manager, this role serves as an operational extension within PIH Health, contributing to a seamless continuum of care across our Whittier network in

The role supports timely, compliant discharge planning and serves as an operational extension of the RN Case Manager under delegated authority.

Required Experience
