Job summary

PIH Health is seeking a Discharge Coordinator to support the Care Management team with forward-facing patient and family engagement, post-acute coordination, and real-time electronic tracking.

The role ensures timely, compliant discharge planning and operates as an operational extension of the RN Case Manager under delegated authority within PIH Health's regional network in Whittier, California.
