Renown Health in Reno, NV is seeking a Part-Time Discharge Nurse (RN) to direct and deliver comprehensive discharge instructions from the discharge suite, ensuring safe, timely patient transitions.

You will educate patients and families, prepare discharge documentation, remove peripheral IV lines, coordinate Meds to Beds, verify Durable Medical Equipment is provided, and collaborate with Case Management/Social Work to overcome discharge barriers.

Qualificações

• Valid Nevada RN licensure required.
• Minimum two years’ experience in an acute care setting preferred.
• English language proficiency (reading, writing, speaking).
• BLS certification required.
• Proficient with Microsoft Office Suite.

Responsabilidades

• Provide discharge instructions and education to patients and families.
• Prepare and deliver discharge documentation.
• Explain discharge plans and educate on medications.
• Remove peripheral IV lines prior to discharge.
• Coordinate Meds to Beds and ensure DME is provided.
• Collaborate with Case Management/Social Work as needed.
• Respond to patient or family distress and activate appropriate team.
• Deliver safe, timely discharges from inpatient units to discharge lounge.
