Boston Medical Center is seeking a Licensed Practical Nurse (LPN) for the Discharge Lounge to support safe, efficient patient transitions from hospital to home or another care level. You will monitor patients, coordinate transport, and collaborate with pharmacy and units to ensure complete discharge processes.

The role requires MA-licensed LPN, BLS, and strong teamwork with a focus on safety and patient satisfaction in a fast-paced environment.

Formación

• Graduate of an accredited LPN program.
• Current Licensed Practical Nurse (LPN) license in good standing in the Commonwealth of Massachusetts.
• Basic Life Support (BLS) Healthcare Provider certification.

Responsabilidades

• Receive and support patients in the Discharge Lounge from inpatient units and ambulatory care settings.
• Monitor patients awaiting transportation, medications, or transfer, and report changes in condition.
• Coordinate patient transportation and ensure discharge requirements are completed.
