Job summary

Capital Health is seeking a Registered Nurse to support admission, discharge, and transfer processes across units. You will maintain open communication with the care team to ensure safe and efficient patient flow, coordinating education and discharge planning.

This role requires attention to detail and collaboration with the throughput team. Ideal candidates have BSN preferred and four years of acute care experience, with BLS and NJ RN credentials.

Qualifications

• Education: Graduate of an accredited school of nursing. BSN preferred; RN required.
• Experience: Four years in an acute care setting; telemetry experience preferred.
• Credentials: AHA BLS Healthcare Provider; RN NJ.

Responsibilities

• Conducts comprehensive assessments of patients on admission and documents in the EHR.
• Collaborates and communicates with the primary nurse on admission needs.
• Ensures all home medication lists are accurate and up to date in the EHR.
• Orients patient and family to room, hospital, and nurse call system.
• Provides and documents education to patient and families.
• Works with throughput team to assist with early discharges.
• Collaborates on discharge needs and provides discharge education.
• Ensures all admission and discharge tasks are complete prior to discharge.
• Documents discharge instructions and education in the EMR.
