Job summary

Ocean Ridge Post Acute in Long Beach, CA seeks a part-time Discharge Planner to organize patient transfers and coordinate medical records in line with OBRA/HIPAA guidelines.

You will support the MDS Coordinator, ensure complete charts, file records, and communicate with interdepartmental teams to ensure timely, accurate discharge planning and compliant information handling. Experience with medical records and HIPAA is desirable.

Qualifications

• Must have a high school diploma or GED.
• Typing speed of at least 45 words per minute.
• Knowledge of medical terminology, anatomy and physiology, and health information.
• Ability to read, write, speak and understand English.
• Ability to apply basic math concepts to practical tasks.
• Familiarity with HIPAA and OBRA guidelines is helpful.

Responsibilities

• Assist in organizing, planning and directing the medical records department.
• Retrieve and file resident charts and records in accordance with policies.
• Assist MDS Coordinator with scheduling assessments.
• Maintain confidentiality and ensure records do not leave the chart room.
• Prepare correspondence and transcribe reports as needed.
