Job summary

Ocean View Psychiatric Hospital in Long Beach, CA seeks a Case Manager/Discharge Planner to coordinate discharge plans, housing placement, and social work support in an 8‑hour shift inpatient setting. You will partner with the treatment team, agencies, families, and community resources to ensure seamless transitions and continuity of care for patients.

Qualifications include a bachelor’s degree in psychology or related field and 2+ years of direct mental health care experience; bilingual

Qualifications

• Bachelor’s degree in psychology, human services, social work, or counseling from an accredited college or university.
• Two or more years direct service experience with severely and persistently mentally ill patients in inpatient or outpatient settings.
• Demonstrates competencies in mental/behavioral health and ability to communicate in English.
• Current CPR, BLS, and First Aid certification or ability to obtain within 90 days.

Responsibilities

• Coordinate discharge plans closely with the patient's treatment team.
• Make accurate referrals for patient placement, case management, and aftercare services.
• Bridge the gap between hospital and referral sources for smooth care transitions.
• Maintain knowledge of community resources and assist with admissions and discharge steps.
• Collaborate with social services and physicians to ensure continuity of care.
• Write intake notes and complete daily progress notes and weekly summary notes on time.
