Job summary

Charlotte Health & Rehabilitation Center in Charlotte, NC is seeking a qualified candidate to join our growing team as the Discharge Planning & Social Work Assistant. You’ll help ensure smooth transitions from hospital to home and collaborate with the Director to address emotional and practical needs.

You’ll assist patients with accessing community resources, coordinating post-discharge services, and supporting families during discharge planning. Competitive benefits offered.

Qualifications

• Bachelor’s degree in social work, psychology, counseling, or related field.
• Experience in post-acute social work, discharge planning, and patient relations.
• Proficient with Microsoft Office Suite (Word, Excel, etc.).
• Excellent communication and interpersonal skills.
• Highly organized and detail oriented.
• Ability to work independently or as part of a team.

Responsibilities

• Advocate for residents’ rights and preferences, ensuring that discharge plans align with their goals for care and community integration.
• Collaborate closely with interdisciplinary teams to develop and facilitate individualized discharge plans that promote patient well-being and autonomy.
• Coordinate post-discharge services, including home health care, community resources, and follow-up appointments, to support patients’ successful transition back into the community.
• Assist patients in accessing and utilizing community resources, such as transportation services, support groups, and recreational activities.
• Provide emotional support and companionship to patients, particularly those experiencing loneliness, anxiety, or depression.
• Other tasks to support the successful operation of the healthcare center.
