Norfolk Health & Rehabilitation Center in Norfolk, VA is seeking a Discharge Planning & Social Work Assistant to help patients transition safely back to the community. You will collaborate with the Director to address emotional, social, and practical needs and connect families with resources for a fulfilling stay and safe discharge.

Qualifications

• Bachelor’s degree in social work, psychology, counseling, criminal justice, or other human services-related field.
• Experience in post-acute social work, discharge planning, and patient relations.
• Proficient with computer programs including Microsoft Office Suite.

Responsibilities

• Advocate for residents’ rights and preferences in discharge plans.
• Collaborate with interdisciplinary teams to develop individualized discharge plans.
• Coordinate post-discharge services including home health care and community resources.
• Assist patients in accessing transportation services and community activities.
• Provide emotional support to patients and families.
• Support the healthcare center's operations.
