Norfolk Health & Rehabilitation Center in Norfolk, VA is seeking a Discharge Planning & Social Work Assistant to ensure safe, smooth transitions for patients as they exit our facility, in coordination with the Director of Discharge Planning & Social Work.

Qualifications

• Bachelor’s degree in social work or related field.
• Experience in post-acute social work and discharge planning.
• Proficient with Microsoft Office.
• Excellent communication and interpersonal skills.
• Highly organized and detail-oriented.

Responsibilities

• Advocate for residents’ rights and preferences, ensuring discharge plans align with goals.
• Collaborate with interdisciplinary teams to develop individualized discharge plans.
• Coordinate post-discharge services and follow-up appointments.
• Assist patients in accessing community resources and transportation services.
• Provide emotional support and companionship to patients.
• Other tasks to support operations.
