Job summary

East Carolina Health & Rehabilitation Center in Greenville, NC is seeking a qualified candidate to join our team as the Director of Social Work and Discharge Planning. Qualifications

• Bachelor’s degree in social work, psychology, counseling, criminal justice, or other human services related field.
• Experience in post-acute social work, discharge planning, and patient relations.
• Proficient with computer programs including Microsoft Office Suite (Word, Excel, etc.).
• Excellent communication and interpersonal skills.
• Highly organized and detail oriented.
• Ability to work independently or as part of a team.

Responsibilities

• Advocate for residents’ rights and preferences, ensuring that discharge plans align with their goals for care and community integration.
• Collaborate closely with interdisciplinary teams to develop and facilitate individualized discharge plans that promote patient well-being and autonomy.
• Coordinate post-discharge services, including home health care, community resources, and follow‑up appointments, to support patients’ successful transition back into the community.
• Assist patients in accessing and utilizing community resources, such as transportation services, support groups, and recreational activities.
• Provide emotional support and companionship to patients, particularly those experiencing loneliness, anxiety, or depression.
• Other tasks to support the successful operation of the healthcare center.
