Preferred background includes CNA/EMT, an associate's degree is preferred, and at least one year in a healthcare setting. You will update systems, handle PAC referrals, and assist with discharge packets and related documentation.

Qualifications

• CNA/EMT preferred and medical knowledge helpful.
• Experience coordinating discharge planning and post-acute services.

Responsibilities

• Coordinate with RN case manager and social worker to support discharges.
• Provide clerical support in case management rounds and update systems as directed.
• Manage referrals through web-based systems and share necessary packets.
• Monitor PAC referrals and expedite discharge processes.
• Maintain timely communication about PAC status and bed availability.
• Prepare discharge packets and ensure physician signatures where required.
