Job summary

Medical City Dallas is seeking a Registered Nurse Case Manager PRN to coordinate patient care, manage discharge planning, and partner with physicians and social services to ensure optimal patient outcomes. The role emphasizes evidence-based assessment, appropriate resource utilization, and patient-centered care throughout the hospital stay.

The RN Case Manager will collaborate with the multidisciplinary team to support transitions of care, ensure regulatory compliance, and advocate for patients’

Qualifications

• Registered Nurse with TX state license.
• Associate Degree in Nursing required; Bachelor's in Nursing preferred.
• 3+ years of clinical hospital nursing experience or 2+ years in case management.
• Certification in case management preferred.
• InterQual experience preferred.

Responsibilities

• Perform comprehensive psychosocial, medical, and discharge assessments.
• Coordinate care plan and discharge planning with multidisciplinary team.
• Refer to Social Services when psychosocial risks are identified.
• Ensure safe discharge and transition through effective communication with physicians, patients, and families.
• Document recommendations and care coordination activities for the care team.
• Advocate for patient rights and consider cultural/ religious values in care decisions.
