Responsibilities

• Assess psychosocial risks and coordinate discharge planning.
• Coordinate transitions to the next level of care with the care team.
• Provide resource information and guidance to patients and families for community services and support.
• Collaborate with physicians and RN Case Managers to ensure smooth discharge and care continuity.
