Job summary

Qualifications

• Bachelor or Master of Social Work required; GA licensure preferred.
• Strong communication, assessment and organizational skills.
• Knowledge of community resources and discharge planning processes.

Responsibilities

• Assess psychosocial needs and plan timely discharge.
• Coordinate care across the continuum with care team and family.
• Provide resources and guidance for post-acute placements and services.
• Document care plans and participate in rounds to optimize outcomes.
• Serve as a consultant on social determinants of health and end-of-life support.
