Job summary

The SW Sr will mentor staff, participate in rounds, and help connect patients with community resources, prioritizing patient wishes and autonomy.

Qualifications

• Bachelor's Social Work required or Master's preferred.
• GA LMSW or LCSW license required.
• Minimum 2 years healthcare acute care experience.
• BLS certification required.

Responsibilities

• Lead discharge planning and coordinate complex patient transitions.
• Assess psychosocial risk factors and community resource needs.
• Collaborate with care team to support care progression.
• Document care plans and progress in the medical record.
• Precept and mentor junior social workers and students.
