Job summary

Wellstar Health System is seeking a Care Coordination Social Worker Sr. to assess transitional care needs, coordinate across the continuum, and support patients and families in discharge planning.

You will serve as a consultant on psychosocial factors and connect families with community resources to enable safe transitions. In collaboration with the care team, you will mentor peers, participate in interdisciplinary rounds, and ensure documentation in the EHR reflects the care plan and progress.

Qualifications

• Master's Degree in Social Work required; LMSW in GA (LCSW can be waived)
• GA Lic Clinical Social Worker or Lic Master Social Worker GA required
• BLS certification preferred or Instructor designation

Responsibilities

• Assess psychosocial risk factors and discharge planning needs.
• Coordinate care with PAS, financial counselor, and UM nurse.
• Collaborate with patients, families, and physicians to establish care progression and discharge plans.
• Provide complex disposition planning and timely resource information.
• Mentor and precept junior social workers and students.
