Job summary

Wellstar Health System is seeking a Care Coordination Social Worker Sr. to assess transitional care needs, coordinate across the continuum, and engage with patients and families to ensure discharge plans are met.

The Sr. role provides expertise on psychosocial and discharge needs, partners with RN Care Coordinators, and mentors staff while integrating the patient’s wishes into the discharge plan.

Qualifications

• Bachelor's or Master's in Social Work required (LMSW/LCSW preferred).
• BLS certification required at hire; GA LMSW/LCSW licensure required or eligible.
• Minimum 2 years of healthcare experience in acute care or skilled care settings.

Responsibilities

• Implements discharge planning and provides resource information in a timely and efficient manner for complex patients.
• Identifies and documents barriers for timely disposition.
• Responds to referrals from RN Care Coordinators and physicians.
• Participates in Interdisciplinary Rounds to confirm discharge date and care transition at discharge.
• Mentors new and less senior employees and serves as a preceptor for student interns.
• Supports ongoing professional competency and performance improvement initiatives.
