Qualifications

• MSW required and licensure
• Strong communication and social work assessment skills
• Ability to coordinate care and discharge planning

Responsibilities

• Conduct social work assessments and interviews with patients and families
• Plan and coordinate discharge preparing safety and care post-hospital
• Refer to community resources for financial assistance, counseling, and rehab
• Arrange ambulance transport and coordinate transfers as needed
• Document assessments and interventions in medical records in a timely manner
• Participate in discharge rounds and team meetings and contribute to quality improvement
