Job summary

Qualifications

• Bachelor’s Degree in Social Work (BSW) or related degree in human services.
• Computer proficiency and literacy.
• Knowledge of CMS, Medicare/Medi-Cal.
• Familiarity with Joint Commission, CMS, CDPH requirements.
• Excellent written and verbal communication skills in English.
• Ability to establish and maintain effective working relationships across the organization.
• Ability to facilitate and lead interdisciplinary rounds.
• One year of healthcare experience (preferred).

Responsibilities

• Interviews patients and families to obtain information for social assessments and discharge planning.
• Coordinates with physicians and care teams to support patient understanding and follow-up care.
• Makes referrals to community resources and ensures HIPAA compliance in documentation.
• Assists in discharge planning and post-hospital arrangements as needed.
• Participates in departmental studies and projects as required by Social Service Department.
