Oceans Behavioral Hospital in Waco seeks a Discharge Planner to coordinate discharge planning across inpatient programs, liaising with physicians, therapists, and the nursing team. You will ensure timely, compliant discharge processes and family communication throughout patient care.

Responsibilities include reviewing psychosocial data, scheduling follow-ups, and maintaining accurate discharge documentation within regulatory standards.

Qualifications

• Associate degree with emphasis in healthcare or bachelor’s degree in social services preferred.
• At least one year psychiatric/chemical dependency experience preferred.
• BLS certification and Oceans approved behavioral health de-escalation program required.

Responsabilités

• Functions as patient liaison to coordinate treatments with physicians, therapist, and nursing staff.
• Reviews psychosocial data and completes discharge assessments for the treatment team.
• Schedules appointments, follows up on absentees, and completes continuing care plans.
• Attends treatment team meetings and communicates discharge planning activities to the team.
• Completes referrals and paperwork for all levels of care and arranges follow-up appointments.
