Qualifications

• BS in sociology or psychology required.
• At least 1 year of experience in behavioral health.
• BLS certification required upon hire.
• CPI certification must be obtained within 30 days of hire.

Responsibilities

• Provide clerical support to Case Managers and Social Workers.
• Collaborate with physicians, therapists, treatment team members, and patients to develop effective continuing care plans.
• Assist with telephone calls, faxing, copying, and coordination of discharge plans.
• Present aftercare options to patients and discuss with individuals or families as needed.
• Collect, release, and manage patient information for accurate research and referrals.
• Maintain timely and accurate patient records.
• Coordinate with referral sources and mental health providers.
• Arrange admissions and transfers to hospitals and centers.
• Schedule follow-up appointments for outpatient treatment.
• Attend treatment team meetings professionally and punctually.
• Build relationships with community resources and referrals.
• Research and update provider/resource lists and databases.

• BLS (American Heart Association) certification for healthcare providers required upon hire.
• CPI certification to be obtained within 30 days of hire.

Job- Specific Duties

• Collaborate with physicians, therapists, treatment team members, and patients to develop effective continuing care plans.
• Assist with telephone calls, faxing, copying, and coordination of discharge plans.
• Present aftercare options to patients and discuss with individuals or families as needed.

• Maintain timely and accurate patient records.
• Initiate contact and coordinate with referral sources, mental health providers, and various care facilities to ensure continuity of care.
• Arrange admissions and transfers to hospitals, treatment centers, and other placements.
• Schedule follow-up appointments for outpatient treatment.
• Attend and contribute to treatment team meetings professionally and punctually.
• Build and maintain positive relationships with community resources and referrals.
• Conduct research and maintain updated provider and resource lists and databases.

Placement in the pay range

Placement in the pay range is based on multiple factors, including but not limited to relevant years of experience and qualifications. In addition to base pay, there may be additional compensation available for CMC Phoenix roles, including but not limited to shift differential and other special pay practices, etc. The posted compensation for the position is a reasonable estimate that extends from the lowest to the highest pay that CMC Phoenix in good faith believes it might pay for this particular job, based on the circumstances at the time of posting.
