UChicago Medicine AdventHealth is seeking a skilled Care Manager to provide comprehensive discharge planning and psychosocial support for patients. You will assess needs across inpatient, observation, and emergency settings, develop effective discharge plans, and coordinate with post-acute services to ensure timely transitions.

Collaborating with a multidisciplinary team, you will educate patients and families, document evaluations, and advocate for patient preferences and rights throughout

Responsabilités

• Provides grief counseling, disease adjustment support, crisis intervention.
• Assesses discharge planning needs for inpatient, observation, and ED.
• Reviews medical records to inform transition of care plans.
• Develops discharge plans with contingency plans throughout the hospital stay.
• Coordinates with post-acute services and facilities for continuity of care.
• Participates in multidisciplinary rounds to optimize care delivery.
• Educates patients and families about illness impacts and resources.
• Organizes patient and family care conferences with the team.
• Documents discharge evaluations, barriers, and progression milestones.
• Advocates for patient rights and preferences during hospitalization.
• Communicates with payors for post-acute care authorization.
