Job summary

HCA Houston Healthcare Clear Lake seeks a Social Worker (BSW) to promote patient-centered care by evaluating psychosocial needs and coordinating discharge planning for identified patient populations on Med/Surg units.

You will collaborate with RN case managers, physicians, and insurers to address barriers to discharge, develop appropriate plans, and educate patients and families about community resources and care criteria.

Qualifications

• Licensed Baccalaureate Social Worker (LBSW) required.
• Bachelor’s degree in Social Work required.

Responsibilities

• Promote patient-centered care by evaluating psychosocial needs and coordinating discharge planning.
• Act as liaison between patient and community resources/post-acute care providers.
• Develop and implement timely discharge plans for identified patients, focusing on Med/Surg units.
• Collaborate with RN case managers, nurses, physicians, and insurers to address patient needs.
• Identify barriers to discharge and coordinate post-acute interventions with the care team.
• Educate patients and families about community resources, benefits, and care criteria.
• Facilitate meetings with care teams and families to support patient-centered decisions.
