Responsibilities

• Provide psychosocial assessments and plan discharge in collaboration with care team.
• Coordinate with patient, family and physicians to support transportation, housing and community resources.
• Mentor and precept new social workers and interns as needed.
• Document all psychosocial assessments and care planning in the medical record.
• Participate in interdisciplinary rounds and care transitions to optimize patient outcomes.
