Qualifications

• 2+ care management experience (Preferred)
• 2+ social work experience (Required)
• Accredited Case Manager (ACM) (Preferred)
• Certified Case Manager (CCM) (Preferred)

Responsibilities

• Provides grief counseling, disease adjustment support, crisis intervention and discharge planning support.
• Assesses discharge planning needs for inpatient, observation, or emergency departments to avoid delays.
• Reviews medical records and factors to inform transition of care plan.
• Develops discharge plans and arranges post-acute care and community services.
