Responsibilities

• Provide grief counseling, disease adjustment support, crisis intervention, and goals of care planning.
• Assess discharge planning needs in inpatient, observation, and/or ED to avoid delays.
• Review medical records and factors to inform care transition plans.
• Develop discharge plans with contingency plans for timely care progression.
• Coordinate post-acute care services and facilities for continuity of care.
