Job summary

AdventHealth in Orlando, FL is seeking a Social Work Care Manager to advocate for patients and families, coordinate discharge planning, and ensure timely transitions across inpatient, observation, and emergency departments.

You will perform psychosocial assessments, educate patients and families, connect with post-acute resources, and collaborate with hospital and community partners to support autonomous health care decisions.

Qualifications

• Bachelor's in Social Work (BSW) required.
• Master's in Social Work (MSW) preferred.
• 1+ care management experience preferred.
• 1+ years experience in social work required.
• ACM/CCM certifications preferred.
• Excellent communication and negotiation skills required.

Responsibilities

• Provides patient and family advocacy and supports patient rights during hospitalization.
• Documents discharge planning evaluations, plans, and barriers to progression of care.
• Reviews medical records and coordinates post-acute care and community resources.
• Participates in multi-disciplinary rounds to review patient status and discharge plans.
• Educates patients and families about emotional, social, and financial impacts of illness and care options.
• Collaborates with hospital and community agencies to obtain needed services.
