Job summary

AdventHealth in Orlando seeks a seasoned professional to supervise discharge planning and social services across inpatient, observation, and emergency settings. You will drive care management strategies, lead multidisciplinary rounds, and resolve barriers for complex transitions.

The role requires 1+ year in care management, 2+ years in social work, and strong analytical and communication skills. A Bachelor’s degree is required, with a Master’s preferred.

Qualifications

• Knowledge of medical necessity for appropriate patient status and level of care.
• Knowledge of IMM, MOON, Advanced Directives, Multidisciplinary Rounds, Social Services, Value Based Programs, and other Care Management compliance guidelines.
• Ability to lead others effectively while maintaining appropriate compliance.
• Knowledge of CMS Conditions of Participation for discharge planning.
• Ability to manage multiple tasks and prioritize level of importance.

Responsibilities

• Supervises daily operations of discharge planning and social services in the inpatient, observation, and emergency department.
• Implements corporate Care Management strategies in collaboration with facility management.
• Participates in multidisciplinary rounds, ensuring active staff participation and effective discharge planning and barrier resolutions.
• Provides discharge planning and progression of care expertise to the interdisciplinary team.
• Leverages reports and dashboards to monitor the accuracy of care manager team's documentation and compliance with regulations.
• Facilitates care conferences for complex transitions and placements as needed.
