AdventHealth in Orlando, FL is seeking a Discharge Planning Supervisor to lead daily discharge planning and social services across inpatient, observation, and ED units.

You will collaborate with facility leadership, participate in multidisciplinary rounds, monitor documentation quality, and mentor staff while ensuring regulatory compliance and seamless patient transitions.

Required: Bachelor's degree, 1+ year in direct care management and 2+ years of social work; Master's degree preferred.

Competenze

• Bachelor's degree required; Master’s preferred.
• 1+ year experience in direct care management.
• 2+ years social work experience.
• Knowledge of CMS Conditions of Participation for discharge planning.
• Experience with reports and PC applications.
