Sparrow Hospital in Lansing, MI seeks a licensed social worker to provide education and counseling to patients and families. You'll assess psychosocial needs, coordinate discharge planning, and collaborate with an interdisciplinary team to ensure timely, patient-centered care across the continuum of services.

Responsibilities include risk assessment, care coordination, and facilitating transitions while supporting patient autonomy and rights.

Formación

• Current LMSW licensure by State of Michigan; OR LLMSW licensure (per state requirements).
• Master's Degree in Social Work.
• Ability to interview, assess, organize and problem solve.
• Experience in acute care case management, discharge planning, utilization management preferred.

Responsabilidades

• Provide education and consultation regarding resources and discharge planning.
• Collaborate to develop discharge plan and monitor implementation.
• Assist patients and families to adjust to illness, treatment options, and prognosis.
• Develop long and short-term plans with patient/family to address identified problems.
• Provide emotional support and counseling related to treatment decisions and end-of-life issues.
• Coordinate and facilitate patient care progression across the continuum.
• Identify and resolve delays in discharge and facilitate transfers as appropriate.
