University of Michigan Health-Sparrow is seeking a Medical Social Worker to provide in-home social work services for patients and families in Lansing, MI. You will assess patient needs, coordinate resources, and support staff as part of the home care team.

The role requires a Master’s Degree in Social Work, Michigan licensure, and at least two years in community health or home health. You must have strong interpersonal skills and the ability to work in patients' homes. Per diem options may apply.

Formación

• Current state of Michigan certification/registration or license as a social worker.
• Active American Heart Association BLS Certification preferred.
• Minimum two years in community health/acute care or home health setting.

Responsabilidades

• Provides medical social casework under physician’s plan of care for patients and families.
• Evaluates patient in-home situations and prepares histories to determine services.
• Assesses patient and family needs and facilitates referrals to community resources.
• Supervises Medical Social Worker – Bachelors per Conditions of Participation.
• Maintains clinical records for home visits and conferences.
• Keeps knowledge of community resources and adheres to regulations.
