Job summary

Qualifications

• Associate's or Bachelor's degree in a related field is preferred.
• Experience in discharge planning, case management, or recovery services is desirable.
• Familiarity with behavioral health systems and outpatient treatment models is a plus.
• Strong organizational and communication skills are essential.

Responsibilities

• Coordinate individualized discharge and aftercare plans with clinical teams and community providers.
• Develop customized aftercare plans including outpatient therapy, housing, and sober living referrals.
• Track post-discharge referrals and ensure continuity of services before discharge.
• Communicate aftercare plans to clients and families and obtain agreement.
• Maintain up-to-date lists of community resources and treatment providers.
• Document updates in the EMR and client records; support alumni engagement.
