Job summary

Pinnacle Treatment Centers, Inc. is seeking a Discharge Planner to initiate discharge planning, coordinating aftercare with the treatment team and referral sources.

You will schedule appointments and document plans in the EHR to ensure smooth transitions at discharge. Responsibilities include maintaining relationships with referral sources, assessing resources, and coordinating housing and community services for patients.

Qualifications

• Bachelor's degree required or equivalent experience.
• 3-5 years clinical case management/discharge planning experience.
• Ability to coordinate with treatment teams, referral sources and community resources.
• Experience documenting discharge plans in an electronic health record (EHR).

Responsibilities

• Provides timely discharge planning from the facility, coordinating with Utilization Review.
• Maintains relationships with referral sources and community agencies.
• Assesses patient resources and coordinates transitions to providers or referral sources.
• Documents discharge plans in the EHR and completes final notes within two working days.
• Collaborates with multidisciplinary teams and supports program operations.
