Job summary

Surfpoint Recovery in Brooklyn, NY is seeking a Discharge Planner to support discharge planning across care levels. The role emphasizes coordination with clinical teams, education for patients and families, and seamless transitions to post-discharge services.

Location is in Coney Island area with a standard 9-5 weekday schedule. Experience in substance use treatment and a 4-year degree are required to apply.

Responsibilities

• Provides clinical and psychosocial services to patients and their support systems.
• Coordinates and facilitates development of individualized treatment plans with the treatment team.
• Provides discharge planning information for initial treatment plans during interdisciplinary meetings.
• Collaborates with external agencies to implement discharge plans.
• Coordinates referrals and post-discharge follow-up for outpatient care.
• Educates patients and families psychosocially as scheduled.
• Ensures adherence to OASAS program standards and coordinates with providers for seamless transitions.
