Job summary

• Develops and implements activities within their scope of practice per ISP.
• Coordinates care with client, family, and providers to implement the service plan.
• Maintains comprehensive clinical records and progress toward recovery.
• Collaborates with clinical team to engage and coordinate care across providers.
• Ensures discharge planning begins at admission and is well-documented.
• Handles referrals and AHCCCS/SSI/SSD processes for eligible clients.
