Qualifications

• High School Diploma or equivalent required; Bachelor’s degree in Social Work, Counseling or related field preferred.
• One or more years previous related experience in a behavioral setting preferred.
• Previous experience with the client population served by the facility preferred.

Responsibilities

• Develops and implements activities and/or programs within their scope of practice.
• Implements services to clients in accordance with their ISP.
• Collaborates with client and family to implement an effective service plan and explain options.
• Maintains the client's comprehensive clinical record and progress notes.
• Coordinates care with clinical team and community resources to implement ISP.
• Ensures discharge planning begins at admission and coordinates transition plans.
• Obtains collateral input from providers and supports.
• Coordinates referrals and ensures services are updated monthly.
• Engages in crisis intervention as needed.
• Communicates client information to the treatment team and participates in goal evaluation.
