Archdiocese of San Antonio is seeking a Disaster Relief Case Manager for a temporary 60-90 day assignment. The role supports disaster-affected individuals and families across Uvalde and Zavala Counties with intake, needs assessment, and coordinated services under the Humanitarian Relief department.

You will conduct home visits, deliver emergency distributions, and coordinate housing, benefits, and legal services, while maintaining confidential records and reporting outcomes to leadership.

Qualifikationen

• Bachelor’s degree in Social Work, Human Services, or related field preferred; OR a minimum of 1 year of relevant social service experience and/or disaster response and relief and/or emergency management.
• Reliable transportation; valid driver's license; valid vehicle insurance.

Aufgaben

• Conduct client intake and needs assessment to determine eligibility, immediate needs, and long-term goals.
• Develop individualized service plans addressing housing, employment, documents, and benefits assistance.
• Maintain and report case notes and enter all client’s assistance to agency data platforms.
• Provide direct case management and supportive services, ensuring timely referrals to housing programs, food pantries, medical care, counseling, and legal services.
• Drive the rapid response vehicle to deliver emergency distributions and provide on-site case support.
• Assist clients with completing housing or benefit applications.
• Collaborate with Catholic Charities departments and community partners to ensure wraparound services.
• Prepare weekly and monthly reports on client outcomes, service counts, and community engagement activities.
• Participate in ongoing professional development, trainings, and program meetings.
• Uphold safety, confidentiality, and the organization’s Code of Conduct.
• Adhere to the Mission and Vision in disaster response and relief.
