Job summary

Olmsted County is seeking a Social Worker in Disability Services Assessment to review referrals, provide assessments, and manage cases for State Plan, Waiver, and Medical Assistance programs.

The role involves conducting MnCHOICES assessments and reassessments, determining eligibility for services, and developing service plans, while supporting a diverse, inclusive workplace and helping clients access community resources.

Responsibilities

• Review referrals; gather info on functional abilities and level of care.
• Provide assessment and occasional case management for programs.
• Conduct MnCHOICES assessments and reassessments for eligibility.
• Assess functional needs to determine service eligibility.
• Interview clients to determine financial eligibility and program eligibility.
• Develop and write individual service plans and support plans.
• Document work using state and local programs.
• Provide information on health care access and Medicare/Medicaid services.
• Complete required contacts and paperwork on time.
• Advise clients on community resources, rights, and responsibilities.
