Qualifications

• Bachelor's degree in social work or related field required.
• One year of HCBS experience preferred for MnCHOICES Certification.
• MnCHOICES certification or ability to obtain it.
• Proficiency with technology and strong organizational skills.

Responsibilities

• Coordinate client intake.
• Receive referrals, interview and assess needs.
• Develop and monitor strength-based service plans.
• Determine resources and referrals to community care.
• Develop abuse prevention plans and address vulnerabilities.
• Provide client case management and documentation.
