Job summary

Tri-County Office on Aging in Lansing, MI, is seeking a Social Work Supports Coordinator to help elderly and disabled clients explore options and maintain independence. You will use person-centered planning to develop care plans and coordinate services while advocating for clients to access supports and benefits.

The role includes conducting assessments with a professional team, monitoring Medicaid Waiver services, and ensuring compliance with ACLS/MDHHS standards.

Qualifications

• Bachelor's degree in Social Work required.
• Current Michigan Social Work license in good standing.
• At least 1 year of experience with the elderly or disabled.

Responsibilities

• Assist participants with Medicaid eligibility for Waiver services.
• Develop care plans using person-centered planning.
• Conduct comprehensive assessments of health, function, and supports.
• Coordinate and monitor services per the person-centered plan.
• Maintain accurate documentation and participate in QA activities.
• Provide advocacy to secure services and promote participant choice.
