Job summary

Klamath County is seeking a Bilingual (Spanish-speaking) Services Coordinator to join the Developmental Disabilities Services team. This role provides targeted case management to individuals with Intellectual/Developmental Disabilities (I/DD), develops ISP and annual plans, and coordinates services with family and providers.

Ideal candidate is fluent in Spanish, has a related degree or experience, and can navigate Oregon public service systems, maintain confidentiality, and document progress in

Qualifications

• Bachelor’s degree in behavioral science, social science, or a closely related field, or equivalent experience in human services.
• Valid Oregon Driver’s License.
• Fluency in Spanish (reading, writing, and speaking).
• Knowledge of Oregon public service system for Developmental Disabilities Services.

Responsibilities

• Develop and periodically revise an ISP or Annual Plan based on assessments, specifying outcomes, goals, and actions.
• Assist individuals and families in developing and implementing a support system based on a Needs Assessment.
• Utilize choice advising to help individuals select services, providers, and settings.
• Develop and monitor plans for effectiveness and Medicaid standards.
• Deliver services using person-centered practices and encourage self-determination.
• Maintain caseload of individuals across service elements.
• Document interactions through weekly progress notes and attend supervision meetings.
