Job summary

The Columbus Organization seeks a dedicated Case Manager in Middlebury, VT to coordinate assessment, planning, and monitoring for individuals receiving HCBS services. You will work with the individual's team to ensure goals, safety, and compliance with federal and state requirements.

Responsibilities include planning meetings, data collection, advocating for self-direction, and coordinating with service providers. Travel across Addison County is expected as needed.

Qualifications

• Bachelor’s degree in any field with 1 year of human services experience, or related equivalency.
• Associate degree in human services or related field with 2 years of related experience.
• High school diploma with 3 years of human services related experience.
• Must reside within the county or within 60 miles or 60 minutes of driving time.

Responsibilities

• Coordinate assessment and planning activities for the individual’s services.
• Assist with the ISA planning meetings and follow-up notes.
• Support self-direction planning and budgeting processes.
• Monitor progress and ensure HCBS compliance.
• Coordinate travel for a mobile/distance-friendly case management role.
