Qualifications

• Associate degree in human services and two years of human services related experience.

Responsibilities

• Coordinate and monitor individual-centered service plans to ensure outcomes are met.
• Assist individuals with planning, budgeting, and accessing community resources.
• Facilitate planning meetings and document ISAs, BIPs, and related plans.
• Support self-direction and offer information on rights, grievances, and appeals.
• Ensure compliance with state and federal HCBS requirements.
