Qualifications

• Bachelor’s degree in social work (BSW) or equivalent human service degree, OR Registered Nurse (RN).
• This position requires a valid driver’s license, a reliable vehicle, and proof of insurance.

Responsibilities

• Manage a professional caseload of individuals with developmental and intellectual disabilities within an assigned geographic area.
• Conduct comprehensive assessments through interviews with individuals served, families, guardians, and service providers.
• Develop, implement, and regularly update individualized, person-centered support plans in collaboration with interdisciplinary teams.
• Coordinate and monitor services to align with support plans, funding criteria, and regulatory requirements.
• Participate as a professional member of interdisciplinary teams with analysis and recommendations for service levels and provider selection.
• Evaluate progress and outcomes, recommending adjustments to services as needed.
• Prepare and review purchase-of-service authorizations and related documentation per guidelines and standards.
• Maintain accurate, timely, compliant case documentation using electronic case management systems.
• Serve as liaison with provider agencies, state partners, and community resources to ensure quality and continuity of services.
• Represent the agency in meetings with providers and stakeholders, offering professional insights and coordination.
• Apply ethics, confidentiality, and regulatory compliance in all decision-making.
