Catholic Charities of Southern Missouri, Inc. is seeking a dedicated Support Coordinator to serve individuals with developmental and intellectual disabilities in Sikeston, MO.

You will independently manage a caseload, conduct comprehensive assessments, and develop person-centered support plans to promote stability and quality of life. You will coordinate and monitor services, collaborate with interdisciplinary teams and community partners, and ensure regulatory compliance in all documentation.

Qualifications

• Bachelor’s degree in social work (BSW) or equivalent human service degree.
• Registered Nurse (licensed).
• Valid driver’s license, reliable vehicle, and proof of insurance.

Responsabilités

• Manage a professional caseload of individuals with developmental and intellectual disabilities in an assigned geographic area.
• Conduct comprehensive assessments through interviews with individuals served, families, guardians, and providers, and analyze information to identify needs and eligibility.
• Develop and regularly update individualized, person-centered support plans with interdisciplinary teams.
• Coordinate, authorize, and monitor services to align with plans and funding requirements.
• Maintain accurate, timely, and compliant case documentation using electronic case management systems.
